PERSONAL WELLNESS PROFILE COMPREHENSIVE ASSESSMENT
The Hendrick Corporate Wellness team uses the Personal Wellness Profile program to improve the quality of your life by evaluating your present health status and making suggestions for the future. The profile booklet will ask questions about your medical history, nutrition, stress level, and exercise habits among other categories. The Wellness Team will also input information into the profile booklet gathered from the health screen. Both of these pieces of information are returned to the employee in the form of an 18-page report listing scores of the categories mentioned above. These scores include ways you can improve your wellness while also giving you tips to a healthier lifestyle.
Your clinical data received from the Hendrick Corporate Wellness health screen is as follows: height and weight, body composition test, resting pulse, blood pressure, total cholesterol (finger stick including HDL, LDL, triglycerides, and glucose), sub-maximal step test, lung function test, grip strength, and flexibility. Depending on the number of employees interested in the health screen, there may also be an option for men over 40 to participate in a prostate cancer screening which would require a full blood draw.
Other opportunities that exist in the Hendrick Corporate Wellness Department are follow-up options after the health screen has been administered. They include opportunities like educational classes (ex: diabetes education, tobacco cessation, and weight management), a corporate group summary report, corporate Hendrick Health Club Memberships or reimbursement, or we can design a separate wellness web page specific to your business.

HENDRICK HEALTH PROMOTIONS
At Hendrick Health Club, we are committed to your health and wellness. With the help of our Health Promotions team, we are able to offer several testing options that inquire about your body and how it functions. Using the tests can allow you to reach specific goals of health that you have set for yourself. The following tests have proven very beneficial to our members.
Resting Metabolic Rate (RMR) – A 15-minute test that shows your individual calorie burn rate at rest. The test will show if your metabolism is fast or slow, show a range of calories you can eat in order to reach your goals, and estimates your total calorie expenditure with daily activities and exercise included.
VO2 Test – A quick exercise test done on the treadmill to determine your ideal heart rate range in order to reach your goals. Your goals can be fat loss, cardiovascular fitness, or sports training.

Cholesterol Test – A test that calculates your HDL (good cholesterol), LDL (bad cholesterol), total cholesterol, glucose, and triglycerides by using a small finger stick. Your results may be available before you leave. A 12-hour fast is recommended but not required.
Personal Wellness Profile (PWP) – The PWP compiles data from your answers on a thorough questionnaire and physical tests done on site. The results can be printed in minutes and will be explained through an 18-page print out. The test will also give you suggestions on how to improve your health scores. Included in the PWP are tests such as blood pressure, total cholesterol, HDL, LDL, triglycerides, glucose, grip strength, flexibility, weight, body fat percentage, lung function, and a three minute sub-maximal exercise test.
Onsite Exercise- The Hendrick Corporate Wellness Team and the Hendrick Training Department offers one hour training sessions at your place of employment. Sessions include two classes per week for six weeks. A minimum of 10 employees must participate. We will work with your particular business to tailor a program that fits the majority of needs of the employees.
